Travel Nursing Document Checklist

1. CPR
2. Nursing License
3. Any certifications needed to work in your specialty
4. Social security card
5. Physical (no more than 1 yr old)
6. Signed declination of Hepatitis B vaccination or proof of vaccination/
7. TB skin test (no more than 1 yr old).
8. Proof of immunizations for Mumps, Rubeola, Rubella, and Varicella
9. Two letters of reference preferrable
10. Your travel nursing resume
